Program Participation Waiver & Release of Liability

1. Acknowledgment of Risk

I understand that participation in fitness training, coaching, workshops, and programs provided by No Quit Movement (including No Quit Code, No Quit Kids, and No Quit H.I.I.T.) may involve physical activity, mental challenges, and lifestyle changes that carry inherent risks. These risks may include but are not limited to: muscle strain, injury, illness, or other health-related issues.

I confirm that I am voluntarily choosing to participate and assume full responsibility for any risks, injuries, or damages that may occur.

2. Health Certification

By signing this waiver, I certify that:

• I am physically and mentally capable of participating in the program.

• I have consulted with a physician and received clearance to engage in exercise, nutrition, and mindset training (or I have voluntarily chosen to participate without such clearance).

• I will immediately stop participation and notify No Quit Movement staff if I experience pain, dizziness, or discomfort.

For No Quit Kids: As a parent/guardian, I certify that my child is healthy, capable, and cleared to participate in this program.

3. Release of Liability

In consideration of being allowed to participate in No Quit Movement programs, I hereby release and hold harmless No Quit Movement, its coaches, trainers, staff, affiliates, and partners from any liability, claims, demands, or causes of action related to participation, whether arising from negligence or otherwise.

4. Media Release (Optional)

I grant permission to No Quit Movement to use photos, videos, or testimonials of me (or my child) taken during the program for marketing, social media, and promotional purposes.

5. Code of Conduct

I agree to uphold the values of No Quit Movement, including respect, discipline, and positivity. Disruptive, unsafe, or disrespectful behavior may result in dismissal from the program without refund.

6. Agreement

I have read, understood, and voluntarily agree to the terms of this Waiver & Release. I understand that by signing below, I am giving up certain legal rights, including the right to sue.
